In order to have a procedure performed you will first have to: 1. Submit a personal medical history. AND 2. Designate a primary care physician in your area who can directly follow up on any positive results.
Please allow 48 hours for the processing of your request. You will receive further instruction concerning a location, contact person, and telephone number for self-scheduling of this screening study. The cost is $10.00 for ordering the study plus whatever charges are required by the provider of the test. The results will be sent to either your e-mail or snail mail address. In some instances we may call you or have your designated primary care physician contact you for follow up.
