Healthcare Concepts, Inc. Memphis, TN and Digital Med, San Diego, CA will showcase a revolutionary Intranet solution for home health care agencies at the NAHC conference in Nashville, TN October 5 - 8, 1996.
Developed by Digital Med, the prototype system features components designed specifically for home health care agencies. The system includes application gateways for patients, nurses, physicians, administrators, and other healthcare professionals. Qualified users are able to access medical records, notes, medication profiles, and care plans using web based versions of Healthcare Concepts existing paper forms.
The data entry and display forms encompass the entire patient clinical record ranging from intake and admission, through care planning, medication and teaching, to discharge and outcomes management. Additionally the Web site system allows instant access to medical and drug databases, agency policy and procedure manuals and training guides.
Video conferencing modules will enable the home health nurse to visually communicate with other nurses, physicians, and health care professionals directly from the patient's home. The video and telemedical monitors could also be used so that the professional nurse can conduct patient education sessions, routine physical observation, and vital signs monitoring from a central office location. These additional telemedical visits would not replace the traditional home visit, rather will allow for more frequent, time effective, and cost effective patient care.
Foster P. Carr, MD, (President and founder of Digital Med) will be at the conference to demonstrate the Web-based home health care information system and its telemedical service capabilities. A live hookup with the Internet will show on line capabilities and the video conferencing feature.
